Download Form DS-11 form the Internet. It is the application that must be completed, but not signed for a USA passport replacement. You must sign it in front of the field office official when you submit your application. You need to photocopy your Present ID, like a driver’s licence or government ID. Make sure to copy both sides. When your application has been processed, the original, certified copies of documents will be returned to you.
Evidence is divided into Primary and Secondary. You must provide one type of Primary Evidence of US Citizenship. It can be a valid driver’s license, a current federal identification card or several other types of documents. When you apply for a copy of your birth certificate, you will get one of two types. One type has all of the information on the original certificate. The abbreviated form does not include the name of your parents on it. The abbreviated birth certificate is no longer accepted unless it is accompanied by Secondary Evidence. This new law became effective on April 1, 2011. Applications submitted prior to this day will be processed using the old rules.
Secondary Evidence includes early public records that include your name, date of birth, and place of birth. Preferably, they should have been created within the first five years of your life. These documents must accompany the Birth Record or Letter of No Record, and will be rejected if sent alone.
Baptism certificates, hospital records and census records are among the documents that may be accepted as Secondary Evidence. They do not replace the documents submitted for Primary Evidence, but must be submitted in addition to them. If submitted alone, they are not sufficient and the application will be rejected.
When you visit the offices to apply, bring Present Identification. This can include your current driver’s license or official identification. Bring a photograph of the appropriate dimensions and any other required items. Make sure to bring photocopies of your present identification, including both sides of it.
