Send In Your Passport Application With All Of The Essential Forms
Form DS-11 is only one of the several documents that are needed when you submit your U.S. passport application. This form is required for most requests, particularly if you have never received travel documents. If you have not had one issued in the past 15 years, you should also use this document.
You can obtain a copy of Form DS-11 online, through a local facility or agency for your replacement pass port. You cannot submit this form through the mail because you must sign it in person. It is imperative that you do not sign it until an official is present to witness your signature.
Your paperwork must be submitted to a passport acceptance facility or agency. An acceptance facility includes post offices, clerks of the court or public libraries that have been given this designation. They are allowed to accept your application and to witness your signature. Agencies are in field offices in large cities that are designed to accept and process paperwork.
Expediting companies are licensed to accept and process your application as well as to hand-deliver it to the local Agency for same-day processing. The number of applications that can be processed in this way is limited so it may take longer than one day to complete. There is a cost associated with expediting companies. Make sure to confirm the cost, since same-day service can be quite expensive.
Primary evidence of U. S. citizenship is required in the packet of information that is submitted. Previously issued passports, a Certificate of Citizenship and certified birth certificates are some of the documents that can be used. For the full list of proof that can be used, check with a local facility or online. When you receive the completed travel documentation, your original paperwork will be included. Your documents may be sent in a separate envelope.
Present identification is required to prove your identity when signing documents. There are several options that will satisfy this need, including an expired passport, a valid driver’s license or military identification card. You will not be required to surrender a present identification information but do need to provide a photocopy of both sides. It must be on plain white paper.
If you will be submitting an out of state driver’s license or other form of primary identification, a secondary identification must be provided. It needs to include the issuance date, date of birth, name and a your photograph. This, too, must have both sides copied on plain white paper.
Appropriate identification must be supplied for the person submitting an application for a disabled person who cannot go to the office to sign. The same rules apply including a photocopy of each side of their identification document. Identification must also be provided for a witness who identifies the disabled person’s signature.
A two-inch by two-inch colored photograph of the applicant must be provided when the packet of information is delivered for a replacement USA Passport. Do not attach the photograph to paperwork, since it will be used on the travel document. It should be a recent picture, taken within the last six months. When the U.S. passport application is submitted with all the pertinent documents, the fee must be paid.
